Effective date: July 2026. How US Medical Notes collects, uses, and protects your information.
US Medical Notes ("we," "us") is a telehealth platform that connects patients with independent, US-licensed providers for real clinical assessments. We respect your privacy and are committed to protecting the information you share with us. This Privacy Policy explains what we collect and how we use it. Certain protected health information is also governed by our HIPAA Notice of Privacy Practices.
1. Information we collect

Contact information — your name, email address, phone number, and general location (such as your state).

Health information you provide — the symptoms, history, dates, and other details you submit so a licensed provider can assess your request.

Payment information — processed securely by Stripe, our payment processor. We do not store your full card number on our servers.

Usage information — device, browser, pages viewed, and similar analytics data collected automatically when you use the site.
2. How we use your information

To connect you with an independent, licensed provider and facilitate your telehealth assessment.

To deliver any documentation a provider issues when it is clinically appropriate.

To process payments and, where applicable, issue refunds under our Refund Policy.

To communicate with you about your request, respond to support inquiries, and — with your consent — send service updates or marketing.

To operate, secure, and improve the Platform and to comply with legal obligations.
3. Third parties and service providers
We share information only as needed to operate the service, and we require our processors to protect it. Key processors include:

GoHighLevel — customer relationship management (CRM) and patient communications, including email and SMS.

Analytics providers — to understand site usage and improve performance.
We do not sell your personal information. Independent providers who assess you receive the information necessary to conduct your assessment. We may also disclose information when required by law.
4. SMS and email marketing — consent and opt-out
With your consent, we may send you appointment updates and marketing messages by email and SMS. Consent to marketing is not a condition of receiving a telehealth assessment. You can opt out at any time:

Email: use the unsubscribe link in any marketing email.
Even after opting out of marketing, we may still send you transactional messages about a request you have made.
5. Data retention
We retain personal and health information for as long as necessary to provide the service, meet medical-recordkeeping and legal obligations, resolve disputes, and enforce our agreements. When information is no longer needed, we securely delete or de-identify it.
6. Security
We use administrative, technical, and physical safeguards — including encryption in transit and access controls — designed to protect your information. No method of transmission or storage is completely secure, but we work to protect your data and to promptly address any issues.
7. Your rights
Depending on your state, you may have the right to access, correct, delete, or receive a copy of your personal information, and to limit certain uses. To exercise these rights, contact us using the details below. We will verify your request and respond as required by applicable law.
8. Children
The Platform is intended for adults. Requests concerning a minor must be made by a parent or legal guardian. We do not knowingly collect information directly from children under 13 without appropriate authorization.
