Refund Policy
Effective date: July 2026. Fair and simple — you only pay for a note you actually receive.
These Terms of Service ("Terms") govern your access to and use of the US Medical Notes website and services (the "Platform"). US Medical Notes is a telehealth technology platform that connects patients with independent, US-licensed healthcare providers for real clinical assessments. When a provider determines it is clinically appropriate, the provider may issue legitimate medical documentation such as a doctor's note. We do not sell pre-made, "novelty," or fake notes.
1. Acceptance of these Terms
By accessing or using the Platform, creating an account, or submitting a request, you agree to be bound by these Terms and by our Privacy Policy, Telehealth Informed Consent, and HIPAA Notice of Privacy Practices. If you do not agree, do not use the Platform.
2. Description of the service
US Medical Notes facilitates telehealth assessments. We provide the technology that lets you submit information and connect with an independent, US-licensed provider who reviews your request and conducts a clinical assessment.

The provider exercises independent clinical judgment. A note or other documentation is issued only if the provider determines it is clinically appropriate.

Submitting a request and paying for an assessment does not guarantee any particular outcome or that documentation will be issued.

If no documentation is issued because a provider determines it is not clinically appropriate, you are eligible for a refund under our Refund Policy.
3. Eligibility
You must be at least 18 years of age to use the Platform on your own behalf. If a request concerns a minor, it must be submitted and authorized by the minor's parent or legal guardian, who accepts these Terms on the minor's behalf. You must be physically located in a US state where the participating providers are licensed at the time of your assessment.
4. User responsibilities and accurate information
The quality and safety of a telehealth assessment depend on the information you provide. You agree to:

Provide complete, truthful, and accurate information about your identity, symptoms, and medical history.

Not impersonate any person or misrepresent your relationship to a patient.

Keep your account credentials secure and promptly update any information that changes.
Providing false or misleading information may result in an inaccurate assessment, refusal of service, and cancellation without refund.
5. No doctor-patient relationship with the Platform
US Medical Notes is a technology platform, not a medical practice, and does not itself provide medical care. Any doctor-patient relationship is formed solely between you and the independent, licensed provider who assesses you. The Platform does not control, and is not responsible for, a provider's independent clinical judgment.
6. Payment and refunds
Fees are displayed before you submit payment and are processed securely by our payment processor. You authorize us to charge the applicable fee for the assessment you request. If a provider determines that documentation is not clinically appropriate and none is issued, you are eligible for a refund as described in our Refund Policy.
7. Acceptable use
You agree to use the Platform only for legitimate telehealth assessments. You may not:

Request or attempt to obtain fraudulent, backdated, "novelty," or fake documentation.

Ask a provider to issue documentation that is not supported by an honest clinical assessment.

Use the Platform for any unlawful purpose or to deceive an employer, school, court, or other party.

Interfere with, disrupt, or attempt to gain unauthorized access to the Platform or its systems.
8. Limitation of liability
To the fullest extent permitted by law, US Medical Notes and its affiliates are not liable for any indirect, incidental, special, consequential, or punitive damages, or for any decision made by a third party (such as an employer, school, or court) regarding documentation issued by a provider. The Platform is provided "as is" without warranties of any kind. Nothing in these Terms limits liability that cannot be limited under applicable law. The Platform does not provide emergency services — if you are experiencing a medical emergency, call 911.
9. Governing law
These Terms are governed by the laws of the state in which US Medical Notes is organized, without regard to conflict-of-law principles. Any dispute will be resolved in the courts located in that jurisdiction, except where applicable law requires otherwise.
10. Changes to these Terms
We may update these Terms from time to time. When we do, we will revise the "Last updated" date below and, where appropriate, provide additional notice. Your continued use of the Platform after changes take effect constitutes acceptance of the revised Terms.
