Telehealth Informed Consent

Effective date: July 2026. Please review before your online assessment with a licensed provider.

US Medical Notes connects you with independent, US-licensed healthcare providers who deliver care using telehealth. This document explains what telehealth is and helps you give informed consent to a telehealth assessment. Please read it carefully; by proceeding with a request, you acknowledge that you understand and consent to the assessment described here.

1. What telehealth is

Telehealth is the delivery of health care using secure electronic communication — such as online questionnaires, messaging, phone, or video — instead of an in-person visit. It lets a licensed provider assess your situation remotely, discuss your needs, and, when appropriate, provide documentation or recommendations.

2. Nature of the assessment

An independent, licensed provider reviews the information you submit and conducts a genuine clinical assessment. The provider may ask follow-up questions before reaching a decision. The quality of the assessment depends on the accuracy and completeness of the information you provide, so please be truthful and thorough.

3. Documentation is issued only when clinically appropriate

Any note or documentation is issued only when the provider determines it is clinically appropriate based on your assessment. Requesting or paying for an assessment does not guarantee that documentation will be issued. We do not provide pre-made, backdated, or "novelty" notes. If documentation is not issued, you are eligible for a refund under our Refund Policy.

4. Limitations of telehealth

Telehealth is not for emergencies. If you are experiencing a medical emergency — such as chest pain, difficulty breathing, severe bleeding, or thoughts of self-harm — call 911 or go to the nearest emergency room immediately.

Telehealth has limits. A remote assessment cannot include a hands-on physical examination and may not be suitable for every condition. In some cases, a provider may determine that you need in-person care and may recommend that you see a local clinician. Telehealth is not a substitute for in-person care where in-person care is medically necessary.

5. Provider licensing by state

Providers are licensed in the US states where they practice, and telehealth is regulated at the state level. You will be assessed by a provider licensed in the state where you are located at the time of your assessment. If we do not have a licensed provider available for your state, we may be unable to serve your request.

6. Patient responsibilities

  • Provide accurate, complete, and honest information about your identity, symptoms, and history.

  • Confirm you are located in a state served by a participating provider.

  • Follow the provider's recommendations and seek in-person or emergency care when advised.

  • Ask questions if anything about your assessment or the guidance you receive is unclear.

7. Consent acknowledgment

By proceeding with a request, you acknowledge that you have read and understood this Telehealth Informed Consent, that you understand the nature, benefits, and limitations of a telehealth assessment, and that you voluntarily consent to be assessed by an independent, licensed provider through the Platform. You may withdraw your consent at any time before an assessment is completed, subject to our Refund Policy.
